This agreement outlines the terms and conditions under which I consent to Cheswick Consultants Pty Ltd trading as Mind Yoga Australia storing my personal information, including health information, in a cloud-based environment, in accordance with the Privacy Act 1988 and the Australian Privacy Principles (APPs).

By signing this agreement I understand and agree to the following:

1. Consent to Cloud Storage: I consent to Cheswick Consultants Pty Ltd trading as Mind Yoga Australia storing my personal information, including health information, in a cloud-based storage system.

I understand that this information will be digitised and uploaded to remote servers managed by Cheswick Consultants Pty Ltd trading as Mind Yoga Australia.

2. Purpose of Storage: I understand that my information will be stored for the purpose of Cheswick Consultants Pty Ltd trading as Mind Yoga Australia providing me with healthcare services and managing my health records. I understand that Cheswick Consultants' access to my information is limited to providing data storage and access services.

3. Data Security and Privacy: I understand that Cheswick Consultants Pty Ltd trading as Mind Yoga Australia are obligated to take reasonable steps to protect my personal information from misuse, interference, loss, unauthorised access, modification, or disclosure, in accordance with the APPs. I am aware that there are potential risks associated with cloud storage, including the risk of unauthorised access or data breaches. I understand that my information will only be disclosed in accordance with the law and my prior consent.

4. Access and Control: I understand that I have the right to access and request corrections to my personal information held by Cheswick Consultants Pty Ltd trading as Mind Yoga Australia. I understand that Cheswick Consultants Pty Ltd trading as Mind Yoga Australia maintains effective control over my personal information stored online.

5. Breach Notification: I understand that I will be notified by Cheswick Consultants Pty Ltd trading as Mind Yoga Australia in the event of a data breach involving my personal information, in accordance with the Notifiable Data Breaches scheme.
